This is when the notices were issued by HSE and what they said about Fawley refinery:

March 14 2023: You have failed to provide dates, supported by a competent structural engineering assessment, when the necessary repair work will be completed on structures identified by you as at risk following the collapse of E-106 fin fan.

July 19 2022: You have not taken all measures necessary to prevent major accidents and in doing so, you have failed to ensure, so far as is reasonably practical, the safety of employees and others from the risk of an explosion on Furnace P3-F2 during furnace start-up or when process upsets occur.

July 19 2022: You have not taken all measures necessary to prevent major accidents and in doing so, you have failed to ensure, so far as is reasonably practical the safety of employees and others from the risk of an explosion on Furnace LE-F-104 

July 19 2022: You have not taken all measures necessary to prevent major accidents and in doing so, you have failed to ensure, so far as is reasonably practical the safety of employees and others from the risk of an explosion on Furnace RF-F-101 

July 19 2022: You have not taken all measures necessary to prevent major accidents and in doing so, you have failed to ensure, so far as is reasonably practical the safety of employees and others from the risk of an explosion on Furnace R2-F-201 

May 13 2022: You have not taken all measures necessary to prevent major accidents in that you have not ensured that the pipework serving sulphur units SU3 and SU4 has been appropriately maintained. Failure of the pipework led to the release of a hazardous substance.

June 4 2019: You have not taken all measures necessary to ensure that pipework containing hazardous substances in the area of Seps-1 and Seps-2 have been inspected and appropriately maintained.
